Healthwatch Wirral Discharge Community Co-ordinator role update June 2022

Laura Peter has recently taken over as our Discharge Community Co-ordinator, making calls to patients recently discharged from hospital.

The care call is aimed at reducing the number of people being readmitted to hospital and to signpost them to a wide range of services and support organisations across the Borough as they continue their recovery.

Laura speaks to people to find out what support they might need to help them stay well at home and prevent readmission to hospital.

During these calls Laura also aims to find out if the call recipient is an unpaid carer or cared for by a friend or family member, and whether there are any accessibility issues such as access to e-consult, transport issues etc.

Many patients have recently praised excellent hospital care, pharmacy services, district nurse services and hospice provision, but also raised difficulties around GP access at some practices. Healthwatch Wirral regularly signposts to a range of organisations when needed, including Wirral InfoBank, WIRED PALS, Age UK Wirral and Citizens Advice Wirral.

Feedback shows that these calls not only make people feel valued, supported, listened to and reassured, but also more empowered to seek out more relevant services for their own health and well-being moving forward.
